| Motor Vehicle Lease Agreement |

The attached form is an agreement between State Fleet Management and a customer agency for long-term leasing of motor vehicles. It consists of the following parts:
Lease agreement (with agency name and other specifics); Appendix A: Obligations of Lessor; Appendix B: Obligations of Lessee; Appendix C: Long-term Lease Rates; Appendix D: List of vehicles leased; and Appendix E: Vehicle Replacement Criteria.
NOTE: The lease rates in Appendix C apply to Model Year 2011 vehicles.

The complete Lease Agreement is a binding agreement between State Fleet Management and another State Agency or other government entity. This form is not yet fillable online. Updated 3 December 2008 to reflect changes in Special Use lease rates for the 2009 Model Year.
